Job Description:
Job Description Open Door Family Medical Center provides health
care and wellness programs to individuals and families in need
throughout Westchester, Putnam and Ulster Counties. A pioneer
Federally Qualified Health Center, Open Door provides more than
300,000 patient visits annually and serves nearly 60,000 individual

expansions. RESPONSIBILITIES: Identify and outreach patients with
potential social, emotional, mental health, and/or substance use
needs · Complete initial psycho-social evaluations, mental status
assessments, standardized screenings and safety assessments for
behavioral health and substance use conditions; complete follow-up
assessments to monitor treatment progress · Collaborate with the
Psychiatric Consultant during weekly case reviews · Ensure that
treatment recommendations, changes in clinical symptoms,
side-effects and concerns are communicated between care team
members · Facilitate treatment plan changes for patients who are
not improving as expected in collaboration with the other
Collaborative Care Team members · Provide brief evidence-based
interventions · Provide education regarding mental health and
substance use conditions and the available treatment options ·
Maintain communication with patients to track treatment response,
monitor for changes in clinical symptoms or treatment side effects
and identify other needs as they arise · Assess patients’
medication adherence, and provide support/education to patients
regarding medication adherence; communicate any medication-related
concerns to the other Collaborative Care Team members · Document
progress notes, including symptoms, problems, screening results,
and interventions provided; document time spent on each interaction
· Develop care plans and treatment plans, including self-management
goals, over-arching goals and objectives in collaboration with
patients · Develop relapse prevention plans in collaboration with
patients · Monitor patients’ engagement and outcomes utilizing the
Collaborative Care Registry, extracted from documentation in the
electronic medical record · Refer internally or externally for
needed service including ongoing mental health and substance use
treatment services and other identified support or social services;
actively manage referral coordination including access, engagement
and follow-up · Provide crisis intervention and warm hand off
assessments for high-risk cases · Respond to and incorporate
physical health concerns into ongoing care interventions and
provide health and wellness education for preventative care ·
Assist patients making optimum use of Open Door’s services by
providing information on services and linking patients to desired
services · Engage parents and/or legal guardians in the behavioral
health care management process, while ensuring informed consent is
obtained as required and privacy laws are followed. · Communicate
clearly and compassionately with families to explain diagnoses,
treatment goals, and available supports and services, promoting
active involvement and informed decision-ma REQUIREMENTS: Masters
in Social Work, in Marriage and Family Therapy, or in Mental Health
Counseling Current New York State registration and license
Bilingual in English and Spanish Recent clinical experience or
training Excellent clinical knowledge with mentally ill and
substance abusing population Knowledge of evidenced based treatment
modalities and crisis management Excellent written and verbal
communication skills Experience in working with lower socioeconomic
and multicultural populations Ability to work collaboratively with
other staff, treatment providers, and community agencies Highly
organized, self directed, able to multi-task in a fast-paced
environment Strong computer skills including working knowledge in
Word and Excel, as well as capacity to navigate and enhance
proficiency in these programs Patient centric with empathy,
excellent active listening and communication skills, engaging and
efficient Passionate about providing high quality care to low
income, under-served residents and those lacking access to health
