delivering outstanding prevention, treatment and recovery programs.
We offer a wide range of programs including Acute Care and
Evaluation, Residential Addiction Treatment, Individual, Group &
Family Therapy, Intensive Outpatient programs and Community Support

Providesquality, cost-effective inpatient/outpatient and/or
community care for clientsand families in a target population
across the continuum of care by assuringaccess, providing
assessments and coordinating clinical summary treatment
plandetermining level of care. Assumes acaseload and performs
behavioral health and/or substance use servicesencompassing
community and ambulatory services and including Behavioral
HealthServices as scope of practice.
Providesdirect client care using individual, couple, group and
family therapy specificto each client’s needs as indicated on
Preparesappropriate medical documentation to meet regulatory
requirements of licensingand third party agencies.
Assessesand plans care by evaluating the psychiatric and
psychosocial needs of theclient; organizes and prioritizes care and
treatment and advocates appropriateresources for the client and
Providesdirect crisis intervention to clients and families in
multiple sites in thesystem and in the community.
Providescontinuous education to client and family members and
refers them toappropriate levels of care, to support groups and to
other available resourcesin order to maximize care and facilitate
recovery or treatment process.
Proficientin working effectively across programs for clients
with both mental health andsubstance use disorders.
Negotiateslevel of care and length of stay with managed care
companies; continuallyre-evaluates client’s needs and informs the
third party payers of ongoingchanges in the client’s status.
Workscollaboratively as a team member along the continuum of
care to provide anintegrated delivery of healthcare to all clients
Participatesin administrative activities including, team
meetings, clinical supervision ofstaff and staff education as
designed by the Program Manager. Assumes responsibility for
self-improvementby identifying, communicating and seeking resources
to meet own learning needs.
Master's Degree in Social Work, Counseling, or related
Threeyears’ experience providing direct services to psychiatric
patients and/orsubstance abuse population.
Previous experiencein assessment and diagnosis and individual
and group therapy.
Valid CT Licensed Masters of Social Work if Master's degree is
in Social Work.
Works towards obtaining licensure requirements.
Demonstrated experience using a computer; basic keyboarding
