Develop and implement individualized care plans for post-transplant patients. Monitor patient progress and adjust care plans based on patient needs and doctor's feedback. Educate patients and their families about post-transplant care and necessary lifestyle adjustments. Coordinate with multidisciplinary healthcare teams to ensure comprehensive patient care. Manage patient data and ensure compliance with healthcare regulations and standards. Strong knowledge of transplant procedures and post-care requirements. Excellent communication and interpersonal skills. Proficient in medical software and electronic health records. Ability to work effectively in high-stress environments. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of practice. Certification in Clinical Transplant Coordination preferred.
