Albany Medical Center, a leading healthcare system, is seeking a dedicated and experienced Clinical Nurse III Post Transplant Coordinator to join our innovative team. This role focuses on the coordination of care for patients following organ transplantation operations. The coordinator will ensure seamless integration of patient care from pre-transplant evaluation through post-transplant recovery, facilitating communication among surgeons, physicians, nurses, and other healthcare professionals. Coordinate patient care before and after transplantation. Educate patients and families about process and healthcare maintenance post-surgery. Monitor patient recovery and report progress to the medical team. Manage patient case documentation and maintain comprehensive records. Act as a liaison between the medical staff and the patient's family. Strong organizational and communication skills Proficient in patient education and advocacy Adept at multi-disciplinary team collaboration Ability to handle sensitive patient issues with confidentiality and care Medical Insurance Dental Insurance Vision Insurance Life Insurance Valid RN license CCTC (Certified Clinical Transplant Coordinator) preferred
