Registered Nurse - Hospice - Per Visit Department: Home Care
Hospice Position Summary The Per Visit Nurse is responsible to
provide a patient centered approach, skilled nursing or other
therapeutic services in the home or other appropriate place. The
Per Visit nurse promotes health and prevents both mental and
physical illness, helping patients obtain other necessary community
services. Minimum Qualifications: Graduate from a hospital school
of nursing with National League of Nursing accreditation at time of
graduation or Graduation from a university public health nursing
program approved by the National league of Nursing. Registered
Professional Nurse with a current license in the State of
Connecticut. One year public health nursing experience and/or
