Job Description:
At AccentCare, we are seeking a compassionate and dedicated
Registered Nurse (RN) to join our Hospice After Hours team. In this
crucial role, you will provide end-of-life care to patients during
after-hours, ensuring comfort and dignity in their final stages.
Your ability to offer skilled nursing services outside of regular
business hours is vital to maintain continuity of care and support
for patients and their families. This position requires excellence
in symptom management, emotional support, and communication skills,
aligned with our commitment to enhancing client well-being through
empathetic, personalized home health care. Provide direct patient
care to hospice patients in their home setting during after hours
(evenings, nights, and weekends). Assess, plan, and implement care
strategies that enhance patient comfort and health. Coordinate with
healthcare team members to ensure optimal patient care outside of
regular business hours. Offer emotional and practical support to
patients and their families. Document care in a thorough, accurate,
and timely manner according to company policy and compliance
standards. Strong clinical skills in hospice care Ability to work
independently in a home setting Excellent communication and
interpersonal skills Skilled in crisis intervention and
patient/family support Proficient with healthcare documentation and
reporting systems Medical Insurance Dental Insurance Vision
Insurance 401k Valid RN license in the state of employment
Certification in hospice and palliative nursing (CHPN) preferred,
but not required
