We’re hiring Registered Nurses (RNs) and Nurse Leaders across our Continuing Care division, which includes Residential Services and Community Services (Home Care & Hospice). Residential Services (Skilled Nursing / Rehab) Provide skilled nursing care, clinical oversight, and care coordination in a long?term care or rehab setting. Common RN & RN Leadership focus areas: - Resident assessments and care planning - Medication administration, treatments, IV therapy (as applicable) - Supervision and clinical guidance of LPNs/CNAs - Collaboration with providers and interdisciplinary teams - Family communication and education Community Services (Home Care & Hospice) Deliver professional nursing care in patients’ homes and community settings—supporting recovery, chronic disease management, and end?of?life comfort. Home Care RN focus areas: - OASIS/assessment (as applicable), care planning, and visit-based care - Coordination with physicians and care teams - Patient/caregiver education and disease management Hospice RN focus areas: - Symptom management and comfort-focused care - Emotional support and education for patients and families - Coordination with interdisciplinary hospice team Locations Hiring - Multiple Continuing Care locations across the region (varies by division and need) What We Offer - Competitive pay and shift differentials (where applicable) - Full?time, part?time, per diem options - Comprehensive benefits (medical, dental, vision, retirement) - Tuition reimbursement and career development and progression - Supportive leadership and a mission?driven culture - Opportunities to grow into charge, educator, case management, or leadership pathways (based on division) What You Will Do (Core RN Expectations) Across Continuing Care settings, you will: - Perform comprehensive assessments and document clinical findings - Develop, implement, and evaluate individualized plans of care - Administer medications and treatments per orders and scope - Identify changes in condition and act promptly to ensure safety - Collaborate with interdisciplinary teams to optimize outcomes - Provide education and emotional support to residents/patients and families - Ensure compliance with policies, procedures, and regulatory standards Key Responsibilities (By Setting) - Residential: clinical oversight, delegation/supervision, care conferences, quality/safety - Home Care: visit-based care, coordination, patient/caregiver training, documentation - Hospice: symptom control, psychosocial support, comfort measures, interdisciplinary coordination - Senior Living: wellness support, care coordination, incident response, resident education Qualifications - Current New York State RN license in good standing - Strong assessment, critical thinking, and communication skills - Compassionate, patient-centered approach across the continuum of care - Ability to work independently (especially for home care/hospice roles) Preferred (Not Required) - Skilled nursing or rehab experience (Residential Services) - Home care and/or hospice experience (Community Services) - Case management, wound care, or geriatric experience (helpful across settings) - Valid driver’s license and reliable transportation (for home-based roles) Ready to Apply? Pay Range: $36.00 - $66.01 per hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
