Job Description:
Summary of Job Purpose Responsible and accountable for the
teaching, assessment, safety and monitoring of chronic and acute
hemodialysis patients as well as providing support and education to
the patients family. Functions as an intricate member of the
multi-disciplinary team. Essential Job Responsibilities The
following is a summary of the essential functions of this job. The
employee may perform other duties, both major and minor, that are
not mentioned below; and specific functions may change from time to
time. Maintains a therapeutic environment through ongoing teaching,
and use of sound clinical judgment and intervention in appropriate
response to the patients age, size and medical condition. Performs
and documents initial and ongoing assessment of the dialysis
patient. Implements patient care plans, development of educational
needs and evaluation of patient outcome. Documentation will include
establishment of nursing diagnosis, orders, goals and progress made
toward achieving a positive patient outcome. Recommends patient
care needs to the hemodialysis charge nurse. Ensures patient safety
through recognition and appropriate response to emergency
situations. Sees that cultural practices will be addressed and
supported as long as such practices do not harm others or interfere
with the planned course of medical therapy. Monitors and assesses
patients blood work and adequately studies and treatment and makes
appropriate recommendations to members of the multi-disciplinary
team. Contributes to development of standards of care, policies and
protocols, and assists with QA/QI projects and data collection.
Initiates, monitors patient throughout treatment and terminates
hemodialysis treatment. Participates in patient and/or family
conferences and serves as patient advocate. Supports the
philosophy, purpose and objectives of the Dialysis Department and
the objectives of the Patient Care Services Department. Performs
any other duties as assigned. Essential Job Qualifications
Education: Minimum 2 yr. College degree; preferred 4 yr. College
degree Experience: Minimum 1 yr. Acute care experience; preferred 1
yr. Hemodialysis experience Training/License: NYS Registered
Professional Nurse; BLS Certifications Other: Nephrology nursing
certification preferred, med/surg nursing certification
preferred.
