About the Role: The Internal Medicine Primary Care Clinician plays
a crucial role in providing comprehensive healthcare services to
patients, focusing on the prevention, diagnosis, and treatment of a
wide range of medical conditions. This position requires a deep
understanding of internal medicine principles and the ability to
develop personalized care plans that address the unique needs of
each patient. Clinicians will work collaboratively with a
multidisciplinary team to ensure continuity of care and improve
patient outcomes. The ultimate goal is to enhance the overall
health and well-being of the patient population while promoting a
patient-centered approach to healthcare. This role is vital in
fostering long-term relationships with patients and their families,
ensuring they receive the highest quality of care throughout their
healthcare journey. Minimum Qualifications: Doctor of Medicine (MD)
or Doctor of Osteopathic Medicine (DO) degree from an accredited
institution. Board certification in Internal Medicine or Family
Medicine. Valid medical license to practice in the state of
employment. Strong clinical skills and a commitment to
patient-centered care. Preferred Qualifications: Experience in a
primary care setting. Familiarity with electronic health record
(EHR) system, Epic. Bilingual proficiency in English and Spanish,
ideal but not required. Participation in quality improvement
initiatives. Responsibilities: Conduct thorough patient
assessments, including medical history reviews and physical
examinations. Diagnose and manage acute and chronic illnesses,
providing evidence-based treatment options. Develop and implement
individualized care plans in collaboration with patients and
healthcare team members. Educate patients on health maintenance,
disease prevention, and lifestyle modifications. Maintain accurate
and up-to-date medical records in compliance with healthcare
