Care Nurse Location: New York, NY (Coverage: Brooklyn, Bronx, and
Manhattan) Start Date: 06/16/2025 Job Type: Contract – 13 weeks(40

and partner facilities. Position Summary: We are currently seeking
two experienced Adult Home Care Registered Nurses for an in-home
care position based in New York, NY. This role involves providing
direct care to adult patients in their homes across the boroughs of
Brooklyn, Bronx, and Manhattan. The ideal candidate will have a
solid background in acute care and current experience in adult home
health. Flexible schedules are available, but consistent days are
required. Key Responsibilities: • Provide comprehensive nursing
care to adult patients in their homes across assigned boroughs •
Manage 5–8 patient cases per day with attention to individualized
treatment plans • Perform clinical assessments, monitor health
conditions, and administer medications • Collaborate with
interdisciplinary teams to coordinate patient care • Educate
patients and caregivers on condition management and care techniques
• Document care provided and maintain accurate electronic medical
records • Travel to patient residences throughout the Manhattan and
Bronx boroughs • Maintain infection control and safety standards
Required Qualifications: • Active New York State RN License
(Primary Source Verified) • Current BLS certification (AHA only) •
Minimum of 2 years acute care experience in Med-Surg, Maternal &
Child Care, Critical Care, or Pediatrics • Current experience in
Adult Home Health • Reliable transportation and ability to travel
throughout the boroughs • Strong clinical documentation and
assessment skills • Ability to manage a flexible, yet consistent
