• Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need
• You will implement the plan of care and evaluate the treatment
• You will document the visit electronically and completing documentation within 24-48 hoursursing, LPN, homecare, home care licensed practical nurse
  • NY State -LPN (Licensed Practical Nurse) -License required
  • Minimum of one year Med/surg experience in a hospital or Longterm Care facility required
  • Drivers License and Car required
  • Bilingual Spanish, Mandarin or Cantonese preferred -
